Please share why are you interested in Osteopathic Medicine and why you want to be trained as an osteopathic physician. (1500c)

Please state whether you were involved in any incidents that led to arrest or any criminal process that may not be reflected in your FBI or state criminal background check. Please include all information including, but not limited to matters that were: 1) expunged; 2) not prosecuted (i.e., nolle prosequi); or 3) dismissed. Please include the date, jurisdiction, relevant facts of the incident(s), and final outcome. *

They also ask for a science and other evaluator contact information

"Please provide the name and contact information for the a Science Faculty Evaluator you requested for a Letter of Recommendation in your AACOMAS application. We will use this information to verify your reference as part of your CHSU - COM admissions requirements."
